Responsibilities and Duties
Reviewing and analyzing patient records from the Electronic Medical Record.
Reviewing documentation and providing ICD-10 coding recommendations based on current coding, OASIS, and PDGM Guidelines.
Collaborating with the clinicians to ensure a high level of accuracy of coding.
Work closely with DON in the agency by assisting with each case for the QA, coding, and OASIS review process.
Observing and reporting unusual patterns in data collection and/or lack of adequate documentation for code assignments.
Communicating and working effectively with colleagues from other departments.
Maintaining patient confidentiality and HIPAA information security during employment.
Must be self-motivated, responsive, and committed to communicating with the management/supervisor closely.
Requirements
Must have 1-2 years experience as a "Home health" medical coder.
Must have experience creating the CMS 485 or Plan of Care.
Must have knowledge or background in reading visit Frequency and Duration.
OASIS-E1 knowledge is a must.
Updated in ICD-10-CM.
Must be knowledgeable in navigating Kinnser EMR.
Background with Case managing is a plus.
Must be knowledgeable with visit notes and physician orders.
Audit Oasis which includes (SOC, ROC, Recert, Discharge and transfer)
Knowledge of Medicare guidelines and PDGM ICD 10 codes
Enter missing medications based on the referral documents
Review and check back notes if corrections were made
Approve OASIS documentation when corrections are made
Create CMS 485 / Plan of Care
Creating necessary physicians order.
Audit Skilled Nurse Visits
Send SNV notes/corrections to clinician
Run OASIS check and analysis
Registered Nurse / Nursing degree
Strong knowledge of clinical documentation, regulatory requirements, and quality assurance principles
Ability to work independently and collaboratively
Excellent communication, interpersonal, and analytical skills
