Remote RN: Chronic Care Management (CCM) Documentation Specialist
Role Overview
We are seeking a detail-oriented, licensed Registered Nurse to support a U.S.-based clinic with remote, administrative Chronic Care Management (CCM) documentation. This is a non-clinical, chart-review and documentation role; no direct patient contact, bedside care, or hands-on clinical duties are involved.The RN will review PPO and Workers' Compensation (WC) patient charts to determine whether patients meet established criteria for CCM noting and will draft compliant CCM notes for qualifying patients.
Key Responsibilities
- Review PPO and WC patient charts provided by the clinical team
- Apply CCM eligibility criteria to determine whether a patient qualifies for CCM services
- Draft accurate, compliant CCM notes for qualifying patients, following CMS/CCM documentation and billing requirements
- Flag charts with missing information, ambiguous criteria, or items requiring provider clarification
- Maintain consistent throughput and accuracy across a monthly volume of approximately 1,400–1,500 chart reviews
- Keep documentation timely, organized, and audit-ready
- Active, unencumbered RN license in good standing (California license not required but an advantage)
- Strong working knowledge of chart review and clinical documentation standards
- Comfortable and proficient with computer-based work, EHR platforms, and digital documentation tools
- Sound clinical judgment, able to independently assess whether a chart meets CCM criteria
- Self-driven and able to work independently with minimal supervision
- High attention to detail and accuracy, given the volume and compliance/billing implications of the notes
- Reliable internet connection and a quiet, HIPAA-appropriate
